We do
charge a fee of $20.00 for filling out forms. The fee is to be
paid when dropping off the forms and we have a 48-72 hour turn
around time for form completion. Payment must be made in cash or
check. We do not charge for sports physical forms or handicap
parking forms.
We also charge for copying medical records. Should you wish
to transfer your care or need a copy, please contact Judi at

Prescription
Refills - Our policy is to complete prescription refill requests
within 48 hours of your request. All mail order prescriptions
will be available for pickup at the office within 48 hours.
Local prescription refills will be faxed/called to your pharmacy
within 48 hours. If you live a far distance away or are
physically unable to pick the prescription up, we can mail the
prescription to you.
Billing
- Canyon Medical Center accepts most health insurance plans, and
as a courtesy we will submit your claims to the carrier for you.
Because we are unable to keep up with the vast array of medical
plans and what each one offers, it is you, the patient that has
the responsibility to notify our office in advance if a
procedure or visit to a specialist needs a referral.
Your co-payment and any unpaid balances is expected at the time of check in for your
appointment.
Self-pay patients are expected to pay at least 50% of the visit
at the time of service.

If you are having financial difficulties, please call PDS to
make payment arrangements.
We also accept VISA, MASTER CARD,
and AMERICAN EXPRESS.
